Job Summary
Conduct clinical pharmacotherapy for up to 15 daily patients through in-person and telehealth appointments, optimizing medication regimens for chronic or high-risk conditions. Serve as a clinical lead in population health to support value-based care goals, managing extensive Collaborative Drug Therapy Agreements with physicians. Provide direct patient care, drug information, and cost containment strategies while contributing to the Pharmacy and Therapeutics Committee and Epic optimization efforts. Mentor new clinicians and participate in multidisciplinary teams focused on improving patient quality of life.
Required Qualifications
- Active Washington license (preferred)
- Applicants must be authorized to work for any employer in the U.S.
- Vancouver Clinic is an alcohol and drug-free workplace
- Offers are contingent on successful completion of background screen and immunization requirements
Desired Qualifications
- Preference will be given to candidates with residency training
- 3+ years of experience
- Newer ambulatory-trained pharmacists who are quick learners and have excellent interpersonal skills are also encouraged to apply
