Job Summary
Conduct direct patient care by managing pharmacotherapy for complex patients through up to 15 daily in-person and telehealth appointments. Serve as a clinical lead in population health to optimize medication regimens for chronic or high-risk conditions, ensuring optimal drug use and cost containment. Lead the Pharmacy and Therapeutics Committee, manage Epic optimization of medication orders, and provide drug information and mentoring to new clinicians. Work side-by-side with physicians and advanced practice clinicians to deliver value-based care within a collegial, physician-owned environment in Southwest Washington.
Required Qualifications
- Active Washington license (preferred)
- Applicants must be authorized to work for any employer in the U.S.
- Vancouver Clinic is an alcohol and drug-free workplace
- Offers are contingent on successful completion of background screen and immunization requirements
Desired Qualifications
- Preference will be given to candidates with residency training or 3+ years of experience
- Newer ambulatory-trained pharmacists who are quick learners and have excellent interpersonal skills are also encouraged to apply
