Job Summary
Clinical Pharmacist Specialist Ambulatory Care position at Vancouver Clinic. Be the clinical lead in population health, collaborating with physicians and APPs to optimize medication regimens for patients with chronic or high-risk conditions, through direct patient care (in-person and telehealth) and programs that support value-based care. The role involves managing pharmacotherapy for complex patients under Collaborative Drug Therapy Agreements, participating in medication-use optimization, providing drug information and cost-containment strategies, and mentoring/trainings within a collegial multi-pharmacist team. The position is full-time, located in Vancouver, WA and Portland, OR offices, with a Washington license preferred and a salary range of $150,318–$175,239 depending on experience. Residency training or 3+ years of relevant experience is preferred; strong interpersonal skills and the ability to see up to 15 patients per day are important. Required Qualifications
- Active Washington license (preferred)
- Doctor of Pharmacy or equivalent degree implied by licensure (Pharmacist)
- experience in ambulatory care or residency preferred
- ability to manage pharmacotherapy for complex patients under Collaborative Drug Therapy Agreements
- strong interpersonal and teaching skills
- availability of in-person and telehealth patient care (up to 15 patients/day)
- familiarity with Epic and population health initiatives
