Job Summary
Clinical Pharmacist Specialist will provide direct patient care and serve as a clinical lead in population health, partnering with physicians and clinicians to optimize pharmacotherapy for patients with chronic or high-risk conditions. The role includes managing pharmacotherapy under Collaborative Drug Therapy Agreements, supporting medication optimization to current standards of care, and contributing to value-based care initiatives. The position involves seeing up to 15 patients per day through in-person and telehealth appointments, with a full-time schedule located in Vancouver, WA and Portland, OR. Required Qualifications
