Clinical Pharmacist Specialist - Amb Care

Assess patient health conditions and medication profiles to optimize therapy and goals of care. Monitor treatment plans, document all activities and communications, and participate in professional development and research. Contribute to performance improvement, drug cost reduction programs, and committee activities while educating pharmacy staff, residents, and students. Serve as a clinical resource for providers and patients within a growing ambulatory pharmacy practice. Mon-Fri schedule with three weekends per year. PGY2 residency preferred.
