Job Summary
Conduct comprehensive medication reviews for high-risk, complex patients, collaborating with physicians, nurses, and social workers to optimize treatment outcomes and minimize medication burden. Evaluate therapeutic effectiveness, recommend medication adjustments including deprescribing, and communicate medication education directly to patients and caregivers. Identify barriers to access and affordability while facilitating timely therapy through partnerships with pharmacies and community resources. Participate in interdisciplinary case reviews, support formulary optimization, and lead pharmacy initiatives such as medication adherence programs. May review and approve certain Part B medications, ensuring all activities comply with federal, state, and CMS requirements. Work remotely with approximately 25% of time spent in the field in Riverside, CA.
Required Qualifications
- Graduate or Advanced Degree, Pharm D
- Active Pharmacist License
- CA License or willingness to obtain one
- Minimum of two years of strong clinical pharmacy experience
- Strong clinical background and knowledge of chronic disease states such as diabetes, heart failure, hypertension, Asthma and COPD
- Must be able to work remote & in the field about 25% of the time in Riverside, CA & the surrounding areas
- A background check is required
Desired Qualifications
- Completion of a PGY1 and/or PGY2 residency or equivalent clinical practice experience
- Basic Life Support Certification
- Vaccine Training Certification
- Board Certification- Pharmacotherapy, Ambulatory Care, Geriatrics
