Job Summary
Clinical Pharmacist (Part-Time) at CBH with a focus on safe, effective medication use in a behavioral health hospital setting. Responsibilities include reviewing medication orders for safety and effectiveness; providing clinical recommendations on psychotropic medications and therapies; participating in interdisciplinary treatment discussions; conducting medication regimen reviews; monitoring adverse drug reactions and interactions; assisting with formulary management and medication-use evaluations; supporting medication reconciliation and transitions of care; providing education to nursing staff, providers, patients, and leadership on medication therapy and best practices; supporting regulatory readiness and accreditation activities; contributing to P&T Committee initiatives and quality improvement projects; reviewing/updating medication-related policies, procedures, and protocols; participating in performance improvement activities to enhance patient outcomes and medication safety.
Required Qualifications
- Doctor of Pharmacy (PharmD) degree
- Current and unrestricted Louisiana Pharmacist license
- Strong knowledge of psychiatric medications and behavioral health treatment principles
- Experience in hospital, behavioral health, acute care, or clinical pharmacy practice
- Excellent communication and interdisciplinary collaboration skills
- Strong analytical and problem-solving abilities
- Knowledge of Joint Commission, CMS, medication management standards, and regulatory requirements
- Ability to evaluate complex clinical situations and make evidence-based recommendations
- Commitment to patient safety and continuous quality improvement
- Strong organizational skills and attention to detail
