Job Summary
Clinical Pharmacist provides safe and appropriate pharmacy services for Option Care patients in home infusion/alternate-site settings. Responsibilities include determining patient suitability for home care, organizing patient-specific information, designing and monitoring pharmacy care plans, educating patients and caregivers, dispensing medications, calibrating equipment, ensuring safety and regulatory compliance (DEA, USP), and coordinating care with Nursing. Qualifications include pharmacy degree, active state license, and relevant experience or residency training. Travel up to 5% within-state expected. The role supports CEU participation, medication-use education, and supervision of pharmacy support personnel as needed. Required Qualifications
- Graduation from an accredited School of Pharmacy
- Current, active Pharmacy license as granted by the State Board of Pharmacy
- One of the following: (a) at least one year of experience in sterile IV admixture procedures; (b) knowledge of non-sterile compounding, immunizations and MTM; (c) completion of an ASHP accredited Pharmacy Residency Program; (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy; (e) equivalent experience as approved by VP/SVP of Pharmacy/Clinical Services
