Job Summary
Clinical Pharmacist position providing safe and appropriate pharmacy services to Option Care patients in home infusion/alternate-site settings. Entry-level role responsible for determining patient suitability for home care, designing, monitoring and adjusting patient-specific pharmacotherapy care plans, educating patients, caregivers and staff, ensuring regulatory compliance, and coordinating with Nursing and other professionals. Responsibilities include preparing and dispensing medications, calibrating equipment, managing inventories, and maintaining documentation; requires Doctor of Pharmacy degree, active license, and demonstrated competencies in sterile IV admixture, immunizations, MTM, or completion of ASHP residency/advanced practice experience. Required Qualifications
- Graduate of an accredited School of Pharmacy
- Current, active Pharmacy license as granted by the State Board of Pharmacy
- One of the following: (a) At least one 1 year of experience in sterile IV admixture procedures (b) knowledge of non-sterile compounding, immunizations and MTM (c) completion of an ASHP accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy or (e) equivalent experience as approved by the VP of Pharmacy and/or SVP of Clinical Services
