Job Summary
Clinical Pharmacist position at Option Care Health in Lexington, KY. Responsibilities include determining patient suitability for home care, collecting and organizing patient information, designing, monitoring and evaluating patient-specific pharmacy care plans for home care pharmacotherapy, recommending modifications to prescribed therapy, providing medication-use education to staff and patients, ensuring continuity of pharmaceutical care across home and other settings, preparing and dispensing medications per policy, ensuring proper labeling, compliance with DEA and state regulations, and participating in education and training activities. Requires strong pharmacokinetics knowledge, ability to monitor labs, adherence to safety and quality standards, and experience with inventory and equipment used in home infusion. Travel up to 5% may be required; salary range provided with benefits; supervisory responsibilities noted as applicable. Travel and CEU requirements are included, along with a comprehensive benefits package.
Required Qualifications
- Graduate of an accredited School of Pharmacy
- Current, active Pharmacy license as granted by the State Board of Pharmacy
- One of the following: (a) At least one year of experience in sterile IV admixture procedures (b) knowledge of non-sterile compounding, immunizations and medication therapy management (c) completion of an ASHP accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy or (e) equivalent experience as approved by the VP of Pharmacy and/or SVP of Clinical Services
Desired Qualifications
- PharmD or Doctorate of Pharmacy
- Active State Pharmacy License
- ASHP-accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience
- Experience in sterile IV admixture procedures or non-sterile compounding/immunizations/MTM
