Job Summary
Entry-level Clinical Pharmacist providing safe and appropriate home-care pharmacy services in an alternate setting. Coordinates patient information and therapy plans with Nursing, monitors progress toward goals, educates patients, caregivers, and staff on medication use, designs and evaluates patient-specific pharmacy care plans for pharmacotherapy in home care, ensures regulatory compliance, calibrates and dispenses medications, and facilitates accurate inventory and information exchange across care settings. Travel up to 5% may be required. Requires graduation from an accredited School of Pharmacy and active state license; ASHP residency or equivalent experience preferred for advanced qualifications.
Required Qualifications
- Graduate of an accredited School of Pharmacy
- Current, active Pharmacy license as granted by the State Board of Pharmacy
- One of the following: (a) At least one year of experience in sterile IV admixture procedures (b) knowledge of non-sterile compounding, immunizations and MTM (c) completion of an ASHP accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy or (e) equivalent experience as approved by the VP/SVP of Pharmacy and/or Clinical Services
