Job Summary
Determine patient suitability for home care by assessing medical conditions, medication therapy, and prognosis against Option Care policies. Design, recommend, monitor, and evaluate patient-specific pharmacy care plans while modifying therapy based on monitoring data and laboratory values. Provide medication-use education to staff, professionals, home care patients, and caregivers, ensuring continuity of pharmaceutical care across settings. Prepare and dispense medications, calibrate equipment, and maintain perpetual inventory systems for controlled substances in compliance with state, federal, and USP standards. Respond to drug information requests from providers and patients, utilizing pharmacy support personnel to prioritize workflow and ensure accuracy.
Required Qualifications
- Graduate of an accredited School of Pharmacy
- Current, active Pharmacy license as granted by the State Board of Pharmacy
- At least one 1 year of experience in sterile IV admixture procedures
- knowledge of non-sterile compounding, immunizations and medication therapy management (MTM)
- completion of an ASHP accredited Pharmacy Residency Program
- completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy
- equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services
- Experience providing customer service to internal and external customers, including meeting quality standards for services, and evaluation of customer satisfaction
- Basic level skill in Microsoft Word
- Basic level skill in Microsoft Excel
- Basic skill level in Microsoft PowerPoint
- Willing to travel up to 5 % of the time for business purposes (within state)
Desired Qualifications
- Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy
- Experience in home care, hospital or other clean room setting
- At least 1 year of experience in indirect management of team members, including assisting in the development, training and assignment of work/projects to other team members
