Job Summary
Provide pharmacokinetic consultation by evaluating drug selection, dosing, and patient response using laboratory data. Manage medication therapy for assigned neonatal patients through reconciliation, monitoring, and education. Document interventions, conduct outcome evaluations, and assist in developing pathways and best practices for the population. Mentor residents and students while educating staff and healthcare providers. Ensure compliance with federal, state, and local laws regarding medications and patient safety.
Required Qualifications
- Bachelor of Science in Pharmacy
- Completion of a PGY1 Pharmacy Practice Residency (3 years related specialty experience accepted in lieu of PGY1 Residency)
- Current Pharmacy license in the state of practice
- Computer skills and dexterity required for data entry and retrieval of patient information
- Proficiency with Windows-style applications and keyboard
- Effective verbal and written communication skills
- Ability to present information clearly and professionally to varying levels of individuals throughout the patient care process
- Strong leadership/teaching skills
- Must be able to stand for prolonged periods of time
- Must be able to travel throughout and between facilities

- Doctor of Pharmacy (PharmD)
- Completion of a PGY2 Pharmacy Residency
