Job Summary
Clinical Pharmacist NE (PRN) at UVA Community Health provides medication management within the healthcare team, performing patient assessments, monitoring therapy for appropriateness and interactions, and participating in direct patient care and rounding. Responsibilities include ensuring formulary adherence, documenting activities, promoting medication safety, and delivering patient education; the pharmacist may prescribe/administer medications as allowed by law, and may precept residents and new team members. The role requires Virginia pharmacist licensure, BLS (and ACLS within six months), 1 year of experience or PGY1/2 years in health-system pharmacy, ability to travel between facilities, and a 4-year/BS degree in pharmacy with FPGEC certification as applicable.
Required Qualifications
- Education: 4 Year/Bachelor's Degree required; Bachelor's Degree in Pharmacy, FPGEC Certification
- Experience: 1 year of experience required - Postgraduate year 1 residency or fellowship OR a minimum of 2 years of experience as a registered pharmacist in health system pharmacy
- Licensure: Current Pharmacist licensure in the state of Virginia completed within 30 days of start date; Board of Pharmacy Specialties Certification or other applicable certification must be obtained and continuously maintained within 2 years; AHA Health Care Provider BLS certification required; ACLS certification within six months of hire required
- Other: Ability to drive/travel to multiple facilities/locations as needed
- Competencies: Medication management and safety; patient assessments; direct patient care and rounding; formulary adherence; documentation; education; precepting residents and students; onboarding of pharmacists and team members
- Scope: PRN role may require on-call responsibilities
