Job Summary
Conduct medication therapy reviews, drug utilization reviews, and targeted patient outreach for routine clinical pharmacy initiatives under established protocols. Assess prescription histories and medication lists to identify adherence gaps, statin-use opportunities, and issues requiring pharmacist intervention or escalation. Collaborate with providers, care managers, and pharmacy technicians to support medication safety, affordability, and access while educating patients on care plans. Document clinical assessments and follow-up actions accurately in applicable systems. Perform comprehensive utilization management reviews for Medicare Part B drugs, evaluating medical necessity, prior authorization requests, and step therapy protocols to minimize total cost of care.
Required Qualifications
- Doctor of Pharmacy degree or other pharmacy degree from an ACPE-accredited College of Pharmacy
- Active, unrestricted pharmacist license in the applicable state of practice
- collaborative practice certification, or related advanced credential
- Additional state pharmacist licenses
- willingness to obtain additional state pharmacist licenses as required
