Clinical Pharmacist

Verify and dispense medications, perform clinical medication reviews, and collaborate with physicians and nurses to optimize pharmacotherapy. Conduct medication reconciliation, therapeutic drug monitoring, antimicrobial stewardship support, and patient counseling on medication use and adherence. Document clinical interventions in the electronic health record, contribute to quality and safety initiatives, and participate in education for patients, staff, and learners.
