Job Summary
Clinical Pharmacist provides comprehensive medication management in Internal Medicine, including reviewing patient medication history, educating patients on drug therapy, establishing therapeutic outcomes, developing/modifying plans, monitoring for drug interactions and adverse events, ensuring accurate dispensing, cross-covering in various practice areas, promoting formulary usage and cost-effective regimens, engaging in safety/regulatory activities, and precepting IPPE/APPE rotations and residents. The role emphasizes education materials and continuing education, with a focus on improving patient experience, safety, and clinical/financial outcomes.
Required Qualifications
- Doctor of Pharmacy (Pharm.D) or BSPharm with 3 years hospital experience
- Massachusetts Licensure in Pharmacy
- Preferred: ASHP PGY1 residency recommended
