Clinical Pharmacist II (Per Diem, Days)

Review drug orders, patient profiles, and medical records to evaluate the appropriateness of drug therapy. Select, compound, prepare, and dispense drugs in accordance with departmental procedures, laws, and regulations. Oversee shift workflow and direct the work of technicians and interns. Consult with medical staff, nursing staff, patients, and other hospital personnel regarding drug therapy and serve as a resource for drug information. Maintain patient profile accuracy by performing routine pharmacy profile-nursing medication administration record comparisons. Document all clinical interventions and formulary compliance in the Electronic Health Record. Identify and report adverse drug reactions and medication errors per policy. Initiate or adjust drug dosages, intervals, routes, or laboratory tests under approved medical staff protocols. Assist with quality improvement projects and data collection as assigned.
