Clinical Pharmacist II - Oncology

Evaluate medication orders for accuracy, resolve discrepancies through communication with physicians and nursing staff, and dispense medications while monitoring drug therapy to ensure appropriateness based on therapeutic intent and patient physiology. Prepare IV admixtures and compound sterile products including large volume parenteral solutions and hazardous cytotoxic drugs, assessing feasibility and recommending alternatives when needed. Participate autonomously in patient care rounds, review patient-specific data to develop therapeutic plans, and counsel patients on dosing schedules and adverse effects. Provide functional supervision for support personnel, manage controlled substance inventory, and respond to drug information requests while ensuring compliance with clinical practice guidelines and departmental policies.
