Job Summary
Evaluate medication orders for accuracy, resolve discrepancies through communication with physicians and nursing staff, and dispense medications while monitoring drug therapy to ensure optimum patient outcomes. Assess feasibility of compounding extemporaneous products, prepare IV admixtures including hazardous cytotoxic drugs, and review patient profiles to determine contraindications and dosage appropriateness. Provide functional supervision for support personnel, direct workflow based on workload, and participate autonomously in patient care rounds to develop therapeutic plans. Respond to drug information requests, counsel patients on medication safety, and ensure compliance with clinical practice guidelines and departmental policies. Maintain perpetual inventory of controlled substances and contribute to quality improvement teams and community outreach efforts.
Required Qualifications
- Doctoral degree from an accredited college of Pharmacy
- Equivalent Pharmacy degree from an accredited college of Pharmacy
- PGY-1 or 3 years of direct patient care experience in a hospital setting
- PHARMD - Licensed Pharmacist in the state of practice
- Board certification within one year of eligibility within 1 Year
