Job Summary

Collaborates with physicians, nurses and other healthcare professionals to ensure safe, efficacious, and cost-effective prescribing, dispensing, administration and monitoring of medications. Participates in education programs, performs duties of the Clinical Pharmacist I as needed, evaluates medication orders for accuracy, dispenses medications, prepares IV admixtures and sterile products, ensures safety and compliance with policies, participates in patient care rounds, provides drug information and counseling, supervises support personnel, and contributes to multidisciplinary quality and service improvement efforts.

Required Qualifications

  • Doctoral degree from an accredited college of Pharmacy required or an equivalent Pharmacy degree from an accredited college of Pharmacy required
  • PGY-1 or 3 years of direct patient care experience in a hospital setting required
  • PHARMD - Licensed Pharmacist in the state of practice required
  • Board certification required within 1 year of eligibility within 1 Year required
