Required Qualifications
- Doctoral degree from an accredited college of Pharmacy or equivalent
- PGY-1 or 3 years of direct patient care experience in a hospital setting
- PHARMD - Licensed Pharmacist in the state of practice
- Board certification required within 1 year of eligibility
