Job Summary
Evaluates medication orders for accuracy, resolves discrepancies through communication with physicians or nursing staff, and dispenses pharmaceutical supplies in accordance with operational procedures and federal laws. Independently responds to non-evaluative drug information requests, assesses feasibility for compounding extemporaneous products, and prepares IV admixtures including large volume parenteral mini-bags, TPN solutions, and hazardous agents. Monitors patient drug therapy to ensure optimal selection and minimal adverse reactions, reviews patient profiles for contraindications, and screens dosages based on therapeutic intent. Dispenses controlled substances while maintaining perpetual inventory and provides technical supervision of support personnel to verify medication preparation accuracy.
Required Qualifications
- Doctoral degree PharmD from an accredited college of Pharmacy
- an equivalent Pharmacy degree from an accredited college of Pharmacy
- 1-2 years of direct patient care experience in a hospital setting
- RPH - Registered Pharmacist Licensure in the state of practice Upon Hire
