Job Summary
Manage clinic pharmacy operations by dispensing prescriptions, counseling patients, and optimizing medication therapy through clinical protocols and therapeutic substitution. Collaborate with healthcare providers to ensure safe, cost-effective treatment while utilizing pharmacy software to maintain patient safety standards. Lead outpatient anticoagulation management for patients, including warfarin dose adjustments, bleeding monitoring, and side effect triage. Provide pharmaceutical information to staff, conduct in-service training, and supervise pharmacy technicians and interns. Participate in quality assurance committees and provider team discussions to advance departmental clinical and sustainability goals.
Required Qualifications
- Washington State licensed pharmacist or eligible to apply for licensure and willingness to be licensed as a Washington State pharmacist preceptor
- Basic Life Support
- Ability to work variable hours
- Hand, wrist and finger dexterity required to open a variety of containers and perform manipulations with needle and syringe
- Hearing ability sufficient to clearly interpret phone orders and drug/health information questions
- Skill required to accurately operate typewriter, calculator, adding machine, and computer equipment
- Clear vision required to: precisely measure liquids and solids by sight, correctly weigh solids on a balance, accurately read and interpret orders, medical information, and journals
- Ability to communicate clearly by speaking and hearing in person or on the phone
- Smell and color perception of solutions, capsules, and tablets
- Manual dexterity and strength required to: stand and walk 8 to 12 hours a day, stretch, bend, and reach high and low frequently throughout the workday, lift boxes, freight and supplies, lift and/or carry up to 25 lbs., and push or pull equipment/supplies
- Ability to work inside the clinic facility in offices and patient care areas
- Possesses knowledge and skills required to effectively care for and assist patients in the following age groups: Neonatal (Birth – 30 days), Infant (30 days – 1 year), Pediatric (1 year – 12 years), Adolescent (12 years to 18 years), Adult (18 years – 65 years), Geriatric (65 years – Death)
Desired Qualifications
- Completion of a PGY1 Community Pharmacy or PGY2 Ambulatory Care Accredited Residency
- Board of Pharmaceutical Specialties certification
- Two years of community pharmacy experience
- Bilingual in English/Spanish
