Job Summary
Review, evaluate, and dispense medications accurately and safely, including unit dose, intravenous, total parenteral nutrition, chemotherapy, and investigational treatments. Partner with physicians, advanced practice providers, and nurses to support medication-related treatment decisions, monitor patient drug therapies, and recommend adjustments or cost-effective alternatives. Educate and counsel patients on safe medication use, help identify and prevent medication-related issues, and provide guidance and oversight to pharmacy technicians. Work collaboratively with multidisciplinary healthcare teams to ensure safe, effective, and personalized treatment across clinical settings.
Required Qualifications
- Doctor of Pharmacy (PharmD) degree or equivalent education and training
- Licensure or eligibility for pharmacist licensure in New York State
- Strong clinical judgment and attention to detail
- Excellent communication and patient counseling skills
- The ability to work collaboratively with multidisciplinary healthcare teams
- A commitment to patient safety, quality care, and continuous improvement
- Experience in hospital, oncology, or specialty pharmacy settings
- Doctor of Pharmacy degree
- Master's degree and the equivalent of one (1) year of full-time clinical pharmacy or pharmacy-related experience
- Bachelor's degree and the equivalent of three (3) of full-time clinical pharmacy or pharmacy-related experience
- Incumbents must be credentialed as a member of the organization Medical Staff (Allied Health Professional) within four months of date of hire
