Job Summary
Lead core clinical pharmacy services, including antibiotic stewardship and therapeutic drug monitoring; conduct medication consults and provide drug information support; perform renal dosing adjustments and clinical interventions; verify and dispense medication orders; prepare sterile IV admixtures; educate patients and staff on safe medication practices; report to the Director of Pharmacy and collaborate with nurses, physicians, and other clinical team members; requires a Doctor of Pharmacy (PharmD) and Texas licensure, with IV certification and hospital/inpatient experience preferred.
Required Qualifications
- Graduate of an accredited School of Pharmacy
- Registered Pharmacist in the State of Texas
- IV certification required
- Hospital or inpatient pharmacy experience preferred
- Minimum experience: hospital internship rotation
