Job Summary
Provide comprehensive medication management for patients with chronic conditions through telehealth or face-to-face visits, assessing efficacy, safety, and adherence. Develop individualized care plans in collaboration with an interdisciplinary team, interpreting laboratory data to monitor therapeutic response and adjust medications under collaborative practice agreements. Identify high-risk patients using population health tools, participate in quality improvement initiatives to reduce readmissions, and track clinical outcomes. Serve as the medication expert on the primary care team, communicating interventions via EMR and team huddles while ensuring a continuum of care with external healthcare professionals.
Required Qualifications
- Bachelor of Science in Pharmacy or Pharm.D. degree
- Current and unrestricted Pharmacist license in state of practice (obtainable within 60 days of hire), with the ability to obtain licensure in additional states as needed
- Completion of a PGY1 pharmacy residency or 3+ years of clinically relevant experience
- Ability to obtain Board Certification (BCACP, BCPS, BCGP) within one year of eligibility
- Legal authorization to work in the United States
- Ability to travel up to 20% of the time based on business needs

Desired Qualifications
- 2+ years of experience in an ambulatory care, primary care, or population health setting
- Active Board Certification (BCACP, BCPS, or BCGP)
