Job Summary
Assess patients, develop therapeutic plans, and write prescriptions for outpatients while documenting all work in the practice management system. Partner with physicians, nurses, and behavioral health colleagues during daily huddles to identify high-risk patients requiring detailed review or separate visits. Conduct one-on-one clinical visits with patients assigned by site leads and organize teaching classes as directed by the pharmacy chair. Participate in peer reviews, quality improvement activities, and staff meetings to maintain care standards, while completing required continuing education and maintaining current licensure and immunization status.
Required Qualifications
- Doctor of Pharmacy (PharmD) degree
- Post-degree residency, or Board Certification in Ambulatory Care Pharmacy
- Current Tennessee pharmacist license
- BLS certification
- Physical readiness for an active clinical role: this position is approximately 80% standing, 30% walking, and 20% sitting, with about 10% of time spent lifting, pulling, or pushing patients and equipment (up to 75-100 lbs., with or without assistance)
- Comfort using a computer, photocopier/fax/scanner, and medical equipment as part of daily work
- Comfort working with patients of all ages and pay ranges, including patients with mental illness, infectious diseases, and exposure to blood and body fluids
Desired Qualifications
- A high level of concentration and attention to detail - this role requires close, intermittent focus, as the number of steps and variables involved creates the possibility of errors without careful attention
