Job Summary
Educate and counsel patients, families, and healthcare team members on safe and effective medication use; perform discharge medication reconciliations and resolve dosing or therapy issues based on evidence-based guidelines; reconcile inpatient orders with provider recommendations and patient health status to ensure appropriate discharge orders; collaborate with inpatient case management and outpatient pharmacy services to ensure post-discharge access to medications; participate in quality improvement initiatives such as readmission reduction tracking and medication-discrepancy resolution; involves pharmacy student and resident precepting; must hold a PharmD/clinical training with licensure as a Pharmacist; schedule: PRN, day shift (Orlando, FL).
Required Qualifications
- Licensed Pharmacist (RPH) required
- Bachelor's degree required
- PharmD degree required or considered as part of education
- Work Experience: 1+ hospital experience strongly preferred
- Licensure and Certifications: Licensed Pharmacist (RPH) required
