Job Summary
Conduct daily clinical evaluations for cardiology patients, including treatment assessment, medication management, and drug regimen reviews for indications, dosing, allergies, and interactions. Manage anticoagulation therapy for warfarin and direct oral anticoagulants, while providing medication therapy management services for adherence and disease state control. Collaborate with multidisciplinary teams to optimize drug therapy and educate BAMC staff and beneficiaries on pharmaceuticals. Monitor polypharmacy and high-dose dosing to prevent adverse effects and support residency rotation requirements. Work Monday through Friday from 0800-1700 or Sunday through Saturday 0001-2400, with shifts ranging from 8-12 hours.
Required Qualifications
- Doctor of Pharmacy (Pharm.D.)
- ASHP-accredited post-graduate year one residency training program (PGY1) with Board of Pharmacy Specialties (BPS) board certification
- ASHP-accredited post-graduate year two (PGY2)
- ASHP-accredited PGY-1 with three (3) years of clinical practice experience with Board of Pharmacy Specialties (BPS) board certification
- American Heart Association Advanced Cardiovascular Life Support (ACLS) certification
- Board of Pharmacy Specialties (BPS) Board Certification
- Current, full, active, and unrestricted license to practice as a clinical pharmacist
- Recent hospital clinical experience within the past 2 years post-graduation/training
- Standards of an ASHP-Qualified Residency Preceptor within one year of employment
- Availability to provide support Monday through Friday from 0800-1700 daily
- Availability to provide support Sunday through Saturday from 0001-2400 daily
- Ability to perform services on various shifts ranging from 8-12 hours
- Inclusion of 30 or 60 minutes for unpaid lunch per shift
- Regular duty hours averaging 40 hours per week
- Competence within the clinical discipline for rotation to clinical positions
