Job Summary
Clinical Pharmacist in Cardiology responsible for interpreting medication orders, monitoring therapy, identifying and resolving medication-related problems, and communicating with physicians, nurses, and patients. Provides drug information, participates in therapeutic drug monitoring, maintains patient profiles, ensures accurate dispensing records, educates patients and staff, and supports formulary management and quality improvement initiatives within the Northwestern Medicine pharmacy department.
Required Qualifications
- Active Clinical Pharmacist license in the state of Illinois (IDFPR)
- Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree
