Clinical Pharmacist (Bilingual- Open to Puerto Rican Residents Only)

Job Summary

Conduct patient outreach via inbound calls and complete asynchronous video consultations to deliver Medication Therapy Management services, including comprehensive medication reviews and prior authorization processing. Assess prescribed therapies for adherence issues, adverse reactions, and dosing errors, then document clinical recommendations and follow up with provider offices to ensure implementation. Collaborate with nurses, health coaches, and product teams to establish operational protocols, optimize workflows, and advise on product enhancements while meeting productivity expectations. Work remotely within Puerto Rico as a licensed pharmacist with three years of experience, fluent in English and Spanish, without visa sponsorship.

Required Qualifications

  • Must reside in Puerto Rico
  • Active Pharmacist License with at least 3 years of pharmacy practice
  • Must be fluent in both English and Spanish, with the ability to read, write, speak, and understand both languages proficiently
  • Ability to obtain licensure in additional states as needed
  • Must be able to serve the elderly population in a clear and fluid manner
  • Excellent communication skills with the ability to develop patient rapport
  • Exceptional listening and interpersonal skills
  • Ability to work independently as well as in collaboration with a virtual/decentralized team
  • Ability to preserve/protect confidential information
  • Self-motivated and able to multitask
  • Proficient in Microsoft Teams, Outlook, Word, Excel, and other programs
  • Must be able to attend meetings and engage with patients on video
  • Must be legally authorized to work in Puerto Rico/United States without employer sponsorship; visa sponsorship is not available for this position

  • Licencia vigente como Farmacéutico(a) con un mínimo de tres (3) años de experiencia en la práctica farmacéutica
  • Dominio completo del inglés y del español (leer, escribir, hablar y comprender ambos idiomas con fluidez)
  • Capacidad para obtener licencias profesionales en otros estados, de ser necesario
  • Excelentes destrezas de comunicación telefónica, especialmente para atender de manera clara y efectiva a la población de adultos mayores
  • Excelentes habilidades de comunicación y capacidad para establecer relaciones de confianza con los pacientes
  • Destrezas excepcionales de escucha activa y relaciones interpersonales
  • Capacidad para trabajar de forma independiente y también como parte de un equipo remoto

Desired Qualifications

  • Experience working in digital health programs and/or working for a health startup
  • Experience working with hospitals or health plans on quality improvement
  • Fluency in Salesforce and additional documentation platforms
  • Sales or call center experience is preferred but not required
  • Strong in problem-solving, communications, collaboration, research, and analytics
  • Results-oriented and thoughtful in your work style
  • Able to work autonomously from idea generation through project execution
  • Highly organized, efficient, able to prioritize, and manage a wide range of responsibilities
  • Able to consistently deliver high-quality work on time and in a manner that makes everyone want to work with you
