Job Summary
The Abdominal Transplant Clinical Pharmacy Specialist is responsible for medication management of assigned abdominal transplant patients, monitors and evaluates therapy per health status and guidelines, and provides patient education. Maintains involvement in education of Pharmacy and Medical Residents, contributes to order sets, formularies, and other related documents, provides pharmacokinetic consultation and dosing evaluations, documents outcomes, develops pathways and guidelines for the population, and mentors students and staff. The role supports transitions of care, participates in publications biennially, and adapts to patient population needs; physical demands may require occasional lifting. This position is based in New Orleans, LA, and is on-site per current postings.
Required Qualifications
- Graduate of an accredited School/College of Pharmacy
- Completion of a PGY-1 Residency in Pharmacy Practice and equivalent experience (3-5 years in Solid Organ Transplant) or completion of a PGY-2 Residency in Solid Organ Transplant
- Licensed to practice pharmacy in the State of Louisiana
- Proficient with Windows-style applications; data entry and retrieval of patient information
- Effective verbal and written communication; leadership/teaching skills
- Mentors residents, students, and new staff; participates in education of healthcare providers and patients
