Job Summary
Clinical Pharmacist responsible for patient-specific home-care pharmacy services, designing, monitoring and adjusting pharmacotherapy plans for home care patients, and educating patients, caregivers, and staff. Duties include determining patient suitability for home care, preparing and dispensing medications, ensuring regulatory compliance (DEA, USP), calibrating equipment, coordinating continuity of pharmaceutical care with the home and other settings, and providing medication-use education. Requires licensure, pharmacotherapy knowledge, and experience with IV admixture, sterile/non-sterile compounding, immunizations, MTM, and residency training; travel up to 5% may be required. Entry-level position with responsibilities spanning patient assessment, plan development, education, and collaboration with Nursing and other professionals; emphasizes adherence to safety, quality assurance, and continuing education.
Required Qualifications
- Graduate of an accredited School of Pharmacy
- Current, active Pharmacy license as granted by the State Board of Pharmacy
- One of the following: (a) At least one year of experience in sterile IV admixture procedures (b) knowledge of non-sterile compounding, immunizations and medication therapy management (MTM) (c) ASHP accredited Pharmacy Residency Program, (d) Advanced Pharmacy Practice Experience with accredited school of pharmacy or equivalent experience as approved by VP/SVP
