Job Summary
Assess clinical care needs, develop and review care plans, and deliver evidence-based, person-centred care to clients in their own homes. Collaborate with the care management team to ensure safe, continuous delivery of high-quality clinical care and provide guidance to care staff. Travel between Ballina, Tweed Heads, Gold Coast, Lismore, Kyogle, and Casino to cover leave and attend to urgent referrals. Requires current AHPRA registration, aged care experience, and ability to work independently across multiple sites. Part-time role with flexible working options.
Required Qualifications
- Nursing qualification
- current registration with AHPRA as a Registered Nurse (Division 1, General)
- A current driver's license
- the ability to travel between HammondCare's sites and services
- Passion for and previous experience in client-focused service delivery in aged and community/home care settings
- Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards
- Excellent communication and collaboration skills

- Ability to work effectively in a multidisciplinary team
