Job Summary
Lead and supervise a team of RNs, LPNs, CMAs, and Clinical Care Coordinators delivering high-quality care management services; oversee day-to-day clinical operations, supervise the care team’s performance, manage patient care workflows, and ensure alignment with Advanced Primary Care Model (APCM) initiatives. Directly reports to the Director of Clinical Operations and collaborates with Product, Client Success, Service Delivery, and Leadership teams to ensure high performance, quality outcomes, and strong patient engagement. This is a hybrid position and includes a comprehensive benefits package. Responsibilities include onboarding, training, performance management, coaching, triage and care coordination, staff scheduling, workflow distribution, clinical escalations, ensuring APCM protocol adherence, monitoring metrics (outreach volume, patient engagement, documentation accuracy, care plan adherence), process improvements, cross-functional collaboration, and participating in clinical quality improvement and program implementation initiatives.
Required Qualifications
- Bachelor's degree in Nursing
- Active, unrestricted RN compact license
- 12+ years of clinical experience
- 1–2 years in a leadership or operations management role
- Experience managing RNs, LPNs, CMAs, and Clinical Care Coordinators
- Experience with ambulatory, chronic care, or value-based care environments
- Strong understanding of Care Management, Care Coordination, CCM/APCM workflows, CMS regulations, and quality programs
- Proficiency in EHR systems (Epic, Cerner, eCW or Veradigm)
