Job Summary
Collaborate with providers to individualize drug therapies and perform comprehensive medication therapy reviews to ensure safe transitions of care. Monitor and modify drug regimens, including ordering and interpreting laboratory testing, while providing refill authorization based on adherence and safety assessments. Act as a drug information resource for providers, nursing staff, and patients, ensuring access to specialty medications by assisting with insurer communications. Optimize drug therapy for under-treated patients and educate patients on self-management while addressing barriers to adherence. Precept pharmacy residents and students, present at multidisciplinary conferences, and develop nursing in-services. Participate in collaborative research projects, submit publications, and assist in preparing drug study protocols. Implement departmental policies, evaluate service literature, and develop drug therapy monitoring systems to prevent medication errors.
Required Qualifications
- Graduation from an accredited School of Pharmacy
- State of Washington Pharmacist License
- Two years of experience in pharmacy practice
- Pre Employment Drug Screening
Desired Qualifications
- Doctor of pharmacy (PharmD) degree, or equivalent, from an accredited School of Pharmacy
