Job Summary
Receive and review case referrals, including conducting home visits to determine hospice and home care needs. Assign appropriate personnel to cases, confer with the Medical Director on eligibility, and evaluate quality through record reviews and clinician feedback. Guide clinicians in establishing therapeutic goals, attending interdisciplinary case conferences, and participating in Quality Assessment and Performance Improvement programs. Assist in hiring recommendations, orientation, and developing in-service education plans while promoting professional standards and community outreach. Requires current RN license, BSN preferred, one year of recent nursing experience, and hospice experience preferred. Full-time exempt role with quarterly record reviews and policy development responsibilities.
Required Qualifications
- Current Registered Nurse (RN) license in the state
- Graduate of an accredited school of nursing
- Minimum of one (1) year of experience as a professional nurse within the last three (3) years
- Ability to work effectively within an interdisciplinary team
- Strong understanding of hospice philosophy, death and dying, and pain/symptom management principles
- Excellent observation, nursing judgment, and verbal/written communication skills
- Current CPR certification
- Valid driver's license and insured, reliable automobile in good working condition
- Compliance with accepted professional standards and practices
