Clinical Nurse Transplant

Provides direct patient care activities including assessment, diagnosis, planning, implementation, and evaluation within the standards of nursing care for a kidney/liver transplant program. Works as part of a multidisciplinary team of nurses, physicians, pharmacists, dietitians, social workers, and spiritual caregivers to meet patient needs. Includes extensive orientation specific to the care of transplant patients and the ability to work nights. Develops and documents individualized plans of care, educates patients and families on post-treatment home care and medication administration, promotes patient safety and quality compliance, and supports professional development. The role emphasizes critical nursing skills to assess physical, psychosocial, emotional, cultural, and population-specific needs, with a focus on post-transplant care within a Magnet-certified academic hospital setting.
