Clinical Nurse Transitional Care Unit

Provides direct patient care in the Transitional Care Unit, including assessment, planning, implementation and evaluation within nursing standards; develops and documents individual care plans with goals in collaboration with an interprofessional team and patient/family; educates patients and families on illness management and home care needs, including medication administration; participates in patient safety, quality and regulatory compliance initiatives; proactively communicates the plan of care across the continuum; utilizes critical nursing skills to assess physical, psychosocial, and emotional needs and considers patient values and barriers; role is safety-sensitive; 3x12 shift schedule with every third weekend during nights; RN license required with Nursing Diploma/Associate's degree and Bachelor’s degree preferred.
