Clinical Nurse Transitional Care Unit

Assess, diagnose, plan, implement, and evaluate patient care while educating families on illness management, home care needs, and medication administration. Develop and document individual plans of care with defined goals in collaboration with the interprofessional team, ensuring communication across the continuum of care. Proactively promote patient safety, quality, and regulatory compliance while addressing physical, psychosocial, and emotional needs according to established standards. This safety-sensitive role supports patient-centered care at a Level I Trauma Center, offering a foundation for future specialty nursing careers. Preferred qualifications include a Bachelor's Degree and RN licensure.
