Clinical Nurse Transitional Care Unit

Provides direct patient care activities in the Transitional Care Unit, including assessment, diagnosis, planning, implementation, and evaluation within established nursing standards. Develops, implements, and documents individualized care plans in collaboration with the interprofessional team and patient/family, coordinates care across the continuum, educates patients and families on illness/injury management and post-treatment home care needs, and administers medications. Emphasizes patient and family-centered care, safety, quality, and regulatory compliance; role is safety-sensitive with nights and 3x12 shifts; supported by a potential sign-on bonus.
