Job Summary

Clinical Nurse Manager responsible for the clinical management of an assigned caseload of pediatric patients in home-care, leading and supervising field staff to meet KPI metrics while ensuring compliance with clinical standards and policies. Duties include case opens, scheduling up to 15 in-home supervisory visits per week, documentation and POC/addendum management, medication reconciliation, supply inventory, telehealth on-call duties, clinical education to clients/families/field staff, and reporting of patient progress. Requires Bachelor’s degree, active RN license, and at least 1+ year nursing experience with knowledge of home care, pediatric populations, SCI/TBI/respiratory failure. Proficiency with laptops, Microsoft Suite, and G Suite is necessary; travel to perform in-home visits is required. The role emphasizes collaboration with the Director of Clinical Services and an interdisciplinary team, along with strong organizational and communication skills, in a Monday–Friday schedule with in-home responsibilities.

Required Qualifications

  • Bachelor’s degree
  • Current Registered Nurse (RN) license in good standing
  • Minimum 1+ year nursing experience
  • Knowledge and/or experience in a home care model
  • Knowledge and/or experience with SCI, TBI, Respiratory failure, Pediatric Populations
  • Working knowledge of laptop/desktop PC
  • Proficiency in Microsoft Suite (Word, Excel)
  • Proficiency in G Suite (Gmail, Drive, Docs, Sheets, Google Meet)
  • Ability to follow written instructions
  • Ability to use computers and computer/software programs
  • Ability to communicate expressively and receptively
  • Demonstrated ability to work with an interdisciplinary team (Operations, Clinical, and Field Staff)
  • Superior customer service skills
  • Proven ability to effectively document findings of visit and communication with staff to meet objectives and paint a clear picture of client status
  • Highly organized with the capacity to work under tight deadlines and shifting priorities
  • Travel is required to perform in-home visits
