Job Summary
Manage the clinical Inbox and task list, triaging customer health and wellbeing notes for deterioration while completing Validated Assessment Tools and referrals for Comprehensive Nursing Assessments. Deliver clinical guidance via telehealth and booked sessions, formulate recommendations for clinical reviews, and respond to escalations, incidents, and complaints. Provide subject matter expertise on clinical policies and advance care planning, uphold service delivery under the Aged Care Act, and contribute to quality improvement and risk management initiatives. Maintain AHPRA registration and CPD requirements while promoting best practice within the HomeMade team.
Required Qualifications
- AHPRA Registration as a Registered Nurse
- First Aid certification
- Experience in Home and Community Care, ideally with aged care clients
- Strong clinical decision-making and case management skills
- Excellent communication and customer service skills
