Clinical Nurse Liaison (RN) – Hospice & Community Outreach - Full-Time

Build and maintain relationships with physicians, hospitals, and community partners to grow hospice services while serving as the primary RN for General Inpatient and routine admissions. Evaluate patients for hospice eligibility in collaboration with physicians and the interdisciplinary team, ensuring compliance with all local, state, and federal regulations including Texas BON standards. Develop and execute community outreach initiatives while tracking activity in Salesforce, working independently to support quality patient care across hospitals and patient homes.
