Job Summary
Build and maintain relationships with physicians, hospitals, and community partners to grow hospice services while serving as the primary RN for General Inpatient and routine admissions. Evaluate patients for hospice eligibility in collaboration with physicians and the interdisciplinary team, ensuring compliance with local, state, and federal regulations including Texas BON standards. Develop and execute community outreach initiatives while tracking activity in Salesforce, promoting the organization's mission across healthcare facilities and patient homes. This full-time role reports to the Director of Business Development and requires a current Texas RN license with at least two years of clinical experience.
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing
- Current Texas Registered Nurse (RN) license
- Current CPR certification
- Minimum of 2 years of RN experience in a clinical setting
- Valid Texas driver's license
- Reliable transportation
- Current auto liability insurance
- Current TB testing
- Hepatitis documentation
- Ability to pass a criminal background check

- Hospice or home health experience
- Experience with timely, compliant documentation
- Proficiency with Salesforce/CRM
- Strong clinical assessment, nursing, communication and relationship-building abilities
