Job Summary
Build and maintain relationships with physicians, hospitals, and community partners to grow hospice services while serving as the primary RN for General Inpatient and routine admissions. Evaluate patients for hospice eligibility in collaboration with physicians and the interdisciplinary team, ensuring compliance with all local, state, and federal regulations including Texas BON standards. Develop and execute community outreach initiatives while tracking activity in Salesforce, working independently to manage priorities across hospitals, healthcare facilities, and patient homes. Requires current Texas RN license, CPR certification, and minimum two years of clinical experience with hospice or home health background preferred.
Required Qualifications
- Graduate of an accredited Diploma, Associate, or Bachelor's School of Nursing
- Current Texas Registered Nurse (RN) license
- Current CPR certification
- Minimum of 2 years of RN experience in a clinical setting
- Valid Texas driver's license
- Reliable transportation
- Current auto liability insurance
- Current TB testing
- Hepatitis documentation
- Ability to pass a criminal background check
