Clinical Navigator - FT Days | Jacksonville Specialty

Cultivate referral relationships, manage assigned territory, and complete patient assessments. Make sales contacts and analyze referral and admission data to facilitate referral conversion with admissions staff, nursing, and internal/external teams. Provide feedback and recommendations for program development and quality improvement initiatives related to customer service and the referral process. Develop and maintain excellent relationships with stakeholders including prospective patients, family members, physicians, ICU and floor nurses, discharge planners, case managers, and payer representatives. Work under the direction of the Director of Business Development to support the shared mission of improving lives through post-acute care.
