Clinical MDS Coordinator

Coordinate Swing Bed program within the hospital, overseeing admission/transfer processes, completing MDS submissions, and managing daily physical assessments. Facilitate EMR documentation and patient/family education, support care coordination across an interdisciplinary team, and promote value-based care through outcomes management. Maintain regulatory compliance with CMS and CIHQ; participate in committees and task forces; translate data into practice and support reimbursement analyses. Serve as a resource for patients and families, help ensure appropriate placement and level of service, and lead ongoing quality improvement and policy development within the Swing Bed program.
