Job Summary
Manage patient discharges by consulting with patients, families, and healthcare teams to develop post-hospital care plans and arrange medical equipment delivery. Conduct daily rounds on nursing units to review discharge needs, monitor admissions, and complete progress notes while compiling monthly statistics for Home Health standards. Supervise assigned personnel through performance management, training sessions, and scheduling, addressing complaints and coaching staff not meeting expectations. Assist in recruitment, selection, and orientation of new employees while participating in hospital committees.
Required Qualifications
- Bachelor's Degree in nursing or equivalent combination of education/related experience
- Registered Nurse (RN) or Physical Therapist (PT) licensure in the state of practice
- Valid Driver's License (DL) and must be at least 21 years of age or older
- Physical Therapist (PT) or Registered Nurse (RN)
Desired Qualifications
- Master's Degree
- Five years' home care nursing experience
- One year's leadership experience
- One year's hospital case management (including discharge planning) experience
- Public Health Nurse licensure in state of practice
- Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy
